The revolt risk mapmode shades by the AVERAGE militancy in a province. Suffice to say that you can easily get revolts in a province that is green in the mapmode if you have the situation that some POPs have high militancy and some low.
In most well run realms with the occasional minor rebellion or with rebel tendencies limited to specific classes or ideologies (that aren't dominant), the rebel mapmode will show up entirely green.
When the mapmode shows large parts of your realm as shades of red, you have got a seriously pissed off populace.

Disappointed to see that in this build, the casualties are similarly meaningless as they are in the demo. Even a huge bloody war like this has no noticeable effect on your soldier POPs or population at large. That's FUBAR.

Releasing a dominion is as simple as opening the political panel and selecting "release nation" on the entity in question. You end up with a loyal (200 relations) satellite.
