Iceland is actually super common in some patches, when Sweden is strong. They conquer all of mainland Norway, so Norway can take the decision to form Iceland.

There are a handful of events to flip provinces Protestant such as 'Bible Translated' where one of the options turns a province Protestant.
My Holland Ironman run has gotten very weird. Right now the League war is ongoing. Kong somehow is in the war despite being Sunni and outside Europe. They have no relations with anyone in either league but are in the war somehow. They do not appear in the HRE screen and at no point did they declare war on me. I was busy fighting the war when I zoomed out and noticed my African colonies were occupied by a purple colored nation. (Spain is in the Catholic league so I was checking my colonies to see if they were occupying them).
Also around 1480 I randomly discovered provinces/regions all over the world. This included all of Japan, a huge chunk of the Caribbean + Colombia and several hordes in Central Asia.
This is in the latest patch with only some cosmetic mods that dont effect checksum. Notice in the upper right that this run is achievement eligible.

Hmmm I could be wrong, but I think this is one of the things that would happen if you click around the later start dates, before going to 1444 and picking a nation?
Usually yes. But in 1.30 this happened with several people who did not do that.

My guess:
Kazan starts as a Sunni nation with a lot of Sunni provinces, they received multiple events where sunni provinces defect to a heretic (Ibadi) Faith. Kazan also starts with a core province of Perm and was forced to release them in a pace deal or lost those provinces to rebels. Result: A newly independent Ibadi Perm
