There is something like a 1 in 3000 chance of it happening every month. I think your character also has to be an adult in order for the chain to start.

It is also significantly more likely if you have some illness and a bit more likely every decade of age you go beyond about 40

That's "winning". I only completed the chain succesfully once before. When my character got cancer, he got an event for being 'miraculously cured' and lost the modifier. You basically receive a get out of jail (death) free card. Not sure if it's just particularly deadly diseases or any disease that triggers it.

Cool, got really lucky then. I successfully cheated once (due to my crafty attribute) and pledged the life of my lover too, but it was worth it it seems
