I played as Germany and completely conquered USA after two years of hard working. But I cannot annex USA and CTD. I can puppet but not annex. Anyone else get this? Do you know how to fix it ?

Are you sure you have all the VPs? Check page 1 of the ledger to see if USA still has some VP provinces left.
Of course. Otherwise Annex option is not appear, isn't it? In SRP 0.6, you cannot puppet USA on 75%. But you can choose either Annex or Puppet when finished all the provinces.
