Somewhat inspired by a mod request thread that got buried a little while ago, I wanted to try and play my realm as something out of Bloodborne, namely Cainhurst.

However, I've been trying to think of where Yharnam and Cainhurst would "be" in our world, as they are somewhat mixed depictions of places. Cainhurst evokes Transylvania, as does the Vileblood bit, but their names (Annalise, Arianna) don't really evoke that area, plus the whole "winter forever" vibe. Yharnam seemed to me to evoke France to me, specifically Southern France and Occitania because of the general style, influence of the Church, and the history of religious conflict preceding the rise of the Church in the area (Healing Church and Hunts and the purge of Vilebloods in BB, Catholic Church and Albigensian Crusade+Inquisition in France).

Any ideas for an "appropriate" location for an in-game allegory of Yharnam and Cainhurst?

If you don't mind modding your game you could make a copy of the file /common/cultures/00_cultures.txt then edit the name list for the Vlach culture (the guys that usually inhabit Transylvania) to be whatever you want.

Don't forget to make that copy and hide it somewhere though, otherwise you will have to repair your game to be able to play Ironman again.
