I don't know, requirement completely controls is the jure or only normal controls?
I ask this question because on the picture i have all 4 counties and create duchy of Librunia, but i don't have "check" conditions in the game. Waiting a new year or loading the game does not help.

Completely. So you need the entire duchy as well as all holdings underneath.

So you need to be acutely aware of any barony shenanigans underneath your counties. It's a pain. This situation is most common regarding holy orders, who often own castles underneath counties. So try to avoid being Catholic so your vassals won't be tempted to allow Holy Orders to construct in their lands.
