You probably missed something. (Or you're not achievement-enabled for some reason, or you didn't fulfil one of the other conditions for the achievement, etc etc.)
If you click the button that asks your Chancellor to "Sow Dissent", the counties highlighted in green contain at least one holding that is not part of your realm. This is pretty useful for detecting rogue baronies.
