Minimum effect:
Previously pagan ruler converted by your Christian chaplain has an opinion bonus with you and suffers piety hit for declaring war on you, possibly alliance.

More elaborate version:

1. Something akin to the tutor relationship with some features of honorary title, as long as both live. Similar to honorary title if e.g. a vassal is liege's godparent.
2. Obligation to stand in for a dead biological parent (part of the point of it, actually) but also opportunity to become a parent figure, form a closer bond.
3. AI scripts reflecting reluctance to act against each other (including factions), not as much as with a biological parent but close.
4. Marriage impediment for 'spiritual kinship' — means your godchild can't marry your bio child (which was the case in the middle ages), obviously you can't marry your godchild.
5. A way of honouring people. The Pope would honour some Catholic rulers who are not his vassals.
6. Possibly military alliance, especially when standing in for a dead biological parent in a noticeable way. Perhaps this should mean more when defending against holy wars, jihads and invasions. (Kind of a device for a Duke of Antioch/Tripoli/Edessa/whatever to call in the Kaiser if they have that relationship.)
7. Higher chance to become regent and/or tutor for a young godchild liege whose bio parent is dead. Higher chance of support in elections maybe.
8. Piety/prestige hit for going to war against, imprisoning, mutilating etc.
9. Intervention before the child would change religion for whatever reason (e.g. under six years of age and parent changes religion; tutored by a Gregarious/Diligent tutor of a different religion).
10. (Minor.) Higher chance to flee to each other's court upon becoming unlanded/banished/whatever.
11. 'Honored godfather', 'my godson' in letters.

This could be a pain in the side to code, I know, or to write up, but it could be the Christian thing in lieu of Muslim school or Indian patron deity.
