My ruler had 2 sons. His demesne was only the county of Kiev and the duchy of Kiev. When he died for some reason the titular kingdom of the Ukraine was given up and the Kingdom of Ruthenia was formed. The kingdom was still run by my oldest son.
 
MY culture is Volhynian. My titular kingdom Ukraine became the de jure kingdom Ruthenia
- Now custom kingdoms instantly dejure drift on formation, however only if the dejure kingdom/empire is not created and if you completly control the titles.

It's from new beta patch. Maybe has something to do with your problem.

I did not see this in the beta data changelog listed.

Are you saying that if I form the kingdom of Normandy it will be de jure unless West Francia or Brittany are formed and if either of those two kingdoms are formed my kingdom will convert to one or the other?

It's almost at the bottom of the list. I don't know the details.
 
Sounds more like elective gavelkind tried to grant your second son a kingdom as it will do, but since there was no county to give him, it had to give it to the first born and in doing so got rid of the titular one.
 
I'm not sure this is what is happening. I started with Pruthenia and formed the Kingdom of Pruthenia but my first son was Getting the Titular title and my second son was getting Kingdom of Lithuania (which was never formed and did not exist).. I did not get to see what happen on my death because I was invaded for the Kingdom of Lithuania and lost and Ruthenia took control of the De Jure Kingdom of Lithuania. And the title still doesn't exist, but I lost all my land

After reading the thread: Any de jure duchies you control entirely not a part of an established title will automatically become your kingdom's de jure. Any de jure duchies you do not fully control or which are de jure of an established title will require the normal de jure drift to occur.

So, in my example: If I form the kingdom of Normandy with complete control of Normandy, Anjou and Brittany they will all become de jure of the kingdom of Normandy - unless either the Brittany kingdom or West Francia kingdom exist. If either of those exist, then normal drift occurs in the duchies within the exiting kingdom.

This is a known bug (reported in sub-forum) and it is exactly what OP's situation looks like to me.
