I'd either go overboard and create all 32 custom nations as an alt-history world, or recreate loads of states from antiquity, or just use it to improve and augment converted games, giving sensible ideas etc.
Here's one I'd like to make though:
Assyria: Based around Northern Iraq and part of Syria.
Culture: Syrian (close enough)
Religion: Coptic (again, close enough)
Ideas:
Traditions: +1 Yearly Legitimacy and -5% Tech Cost.
Ambitions: -20% Core Creation Cost.
The Lamassu Guardians: +20% Fort Defence.
As Old as History: -1% prestige decay.
Royal Palaces: -10% build cost.
Church of the East: +2% Missionary strength.
Royal Lion Hunts: +10% Cavalry combat ability.
Fertile Crescent: +10% Production Efficiency.
King of Kings: +2 Diplomatic Relations.
