I thought I had some evil proclivities, but I can see that some of you have me beaten by miles! :O

The worst I due is my habitual persecution of the Dalassenos and Capet dynasties. Way back in the early days of CKII the Capets murdered my favored genius heir, when i was trying for a Karling restoration (before Old Gods). What followed was a multi-generation genocide spanning three countries and several wars, but in the end every single male line descendant of Hugh Capet was no more.

Dalassenos was just frustrating me at every turn, and everytime I thought: NOW they're done, they schemed their way into some position of power. It resulted in the end of the Byzantine empire and the formation of the kingdom of Anatolia under some random dude, but the Dalassenos were no more!

Since then I've hated them with a passion, and make it a point to whipe em out if they're in the game!

I like to keep prisoners in my dungeon to see how long they live. I'll check the dungeons occasionally to see how long the prisoners have been there. I want them to suffer for as long as possible before they die. The longest I've managed to keep someone in prison for was from their birth until they died at around 70.

EDIT: I think I should change my answer after just genociding basically the entire underage female nobility of my kingdom when their parents decided to rebel against me. I'm using the equality doctrine and for some reason all of their heirs were female and they were all in the capital. I captured them and instead of ending the war I executed every last one of them and made the war last years longer than it would have.

Twenty years later, I arrest somebody and wonder why the prisoner tab says there are 2 people. I'd completely forgotten what I was doing earlier! At least she had a rat friend, but I felt SO BAD. (And I immediately released her.)

On my first HF game I choose the Aztec Reformation for the sake of trying it. Needless to say, no prisoner lasted more than a week in my dungeon.

Also sometimes to make things interesting I imprison and execute vassals that hate me to make the others rebel, just for the sake of it

My current game I'm playing as warmongering bloodthirsty reformed zunists, with all of the hundreds of kills that goes with that. I frequently try to kidnap and kill all of the children of a given ruler in order to make them more likely to shatter on succession.
At one point me and a particular Indian ruler who was the only adult member of his dynasty got into a bit of a competition to see if I could make the line die with him by killing his children faster than he could have them. I would go out of my way to siege his capital down every time i got the chance, and even deliberately let him unsiege it only to siege it again on a number of occasions. Both sides made remarkably good efforts, but unfortunately he narrowly managed to win in the end. The final score was something along the lines of:

Women and children get house arrest, men get the dungeon unless they are particularly tough or persistent rebels, in which case they get 6-12 months in the oubliette. I ransom whomever I can, if the liege/husband can't afford the ransom then I will gift them the 10 gold to pay me - the net result is a win-win, they now love me for charging them to release their husband/wife/son/daughter.

Unless they piss me off.

In that case I make full use of the "Prisoner actions do not release prisoner" game rule - my favourite is Torture followed by being dumped head first into the bottle dungeon, in most cases the whimpering stops after about a week and I can scrape them out ready for my next victim.

In most case I also get to pick up the Cruel trait so there's that bonus too !

I couldn't agree more. Even if not selecting the specific method we should at least be able to select between mercifull and brutal excecution. So you could for example group beheading or given poison as mercifull while on the other hand crucified or walled up alive as brutal. Also specific captives should give specific more excecution methods. For example more likely to burn heresiarchs, hang rebel peasants as petty criminals etc.
It really bums me when my cruel character decides to just behead the sceeming vasal that somehow managed to poison his perfectly groomed heir.

I once imprisoned my daughter few times and cut off all her limbs, blinded her and mutilated her face (for some reason I could do it w/o opinion penalty).

She didn't like me after this.
