Just started a new game - 1444, Castile. I didn't notice this in my previous Castile game, so I don't know if this is hardcoded or if I was assigned a random heir. Either way, I've never seen anything like this in years and years of EU experience.
Any bad heir is known as an "Enrique" in the game. Solely because of this useless idiot. The general Castile game starts with a disinherit of this heir and hoping for a female.

It requires some luck to have him die a timely death. Recruiting him as a military leader, assigning him to an army and having him besiege forts are all factors that increase the chance of mortality.
There's a roll for rulers/heirs to die based on their age. Having them a general adds one, leading an army adds another, and I think being in battle adds another too. Drilling may do the same as being in battle.
The same for generals I think, being in battle adds to it- but I don't know if age is taken into account as the game doesn't display it.
