what does it say when you ask them? does it say "likely" or "unlikely"?

the longer they are your vassal the more likely they are to accept. they'll accept eventually but like the guidance says, it's not a 100% thing. you can save and reload and ask them again if you're really impatient.

After 20 years I, as Tuscany, annexed Modena with somewhere around 5 presitge, 10 BB, and 191 relations, without doing any sort of save-and-reload tricks.

It can be done.

Don't use diplomatic options which raise relations if they are already high, save them as cheap fixes for the first couple of rejections.

Helpful advice: be bigger than the country you are trying to dipannex. This means (in order of priority, I think) # of provinces, # of regiments and ships, DIP rating, prestige, manpower, and income. Have your maintenance set at 100%. Have an army and/or fleet on their border. Don't have done something recently which raised your BB rating. And finally (and I don't know if this still applies, but it seemed to in EU1), don't have an army inside their borders and don't have them have one outside.
