New heir is one of my half brothers who isn't her son. Good job, wife!

Yet...

I think the point is that since you would rather have a son of yours inherit, than allow your half-brother to get the title, she is expecting you to warm up the bed.

The plain meaning of "to see her own son declared heir" is someone who currently exists and not some hypothetical future person

Who told you that? It doesn't say a word of a born son, so it might as well be a hypothetical future son she's "planing to produce". It like an ambition, a goal one works to achieve in the future.
Of cause that might not be what the developers meant, but that's up to them to declare.

This is a bug. Have a look at the trigger node for this event:

Code:
### Evil wife plots to kill sons that aren't hers
character_event = {
	id = 89030
	desc = "EVTDESC89030"
	picture = GFX_evt_quarrel

	capable_only = yes
	prisoner = no

	trigger = {
		is_female = yes
		[B]num_of_children = 1[/B]
		age = 16
		is_married = yes
		is_pregnant = no
		religion_group = muslim
		any_child = {
			is_female = no
		}
		spouse = {
			any_child = {
				ai = yes
				NOT = { age = 16 }
				mother = { NOT = { character = ROOT } }
				is_primary_heir = no
				at_location = ROOT
			}
		}
		NOT = { trait = kind }
	}

For this event to fire, the mother must have at least 1 child. However, there is no code to check if that child is alive. It is a simple fix to add such code:

Code:
### Evil wife plots to kill sons that aren't hers
character_event = {
	id = 89030
	desc = "EVTDESC89030"
	picture = GFX_evt_quarrel

	capable_only = yes
	prisoner = no

	trigger = {
		is_female = yes
		num_of_children = 1
		age = 16
		is_married = yes
		is_pregnant = no
		religion_group = muslim
		any_child = {
			is_female = no
                        [B]is_alive = yes[/B]
		}
		spouse = {
			any_child = {
				ai = yes
				NOT = { age = 16 }
				mother = { NOT = { character = ROOT } }
				is_primary_heir = no
				at_location = ROOT
			}
		}
		NOT = { trait = kind }
	}
