In Hinduism, few pagans regions and few cultures was practice burning widows after death husband. Sometimes this was voluntary, sometimes not. Accepting the widow could give a bonuses, if the widow was wife of great lord (eg. MA for kings of unreformed pagans; bonus to the opinion of priests and church taxes). Ofc family of widows will not be happy, new ruler can lose a talented woman etc.
Often death of great ruler was often source of internal chaos. Many vassals in early times of new rule sought the path to independence, the creation of a new party, the introduction of an alternate heir or the beginning of an internal war; neighbors took the opportunity to take revenge or conquer. In EU series we have "-1 stability" but in CK2 not - often succession is peaceful and non problematic. I would introduce chance (eg. 50%) to some punishment to the relationship in early period and/or loss of soldiers in holdings.
Even bigger problems take place after death in battle. So next maluses.

Eg. after Battle of Stilo the Otton II was declared dead so anti-emperor faction was formed, which after a real death, could more quickly try worked and in Polabie had place great slavic riot (which lead to independence Polabie and many destroys in east German settlements).
