Has this happened multiple times in playthrougths or is it just a one off? I think the ai can refuse occasionaly.

First try, but both Czech and Yugoslav refused. So I thought I might be doing something wrong... too low a world tension? (2% czech, 27% Yugoslav). Are there any conditions on the AI event or is it just a random roll?

Looking at the event files the AI should have a 90% chance of joining, so you're probably just unlucky.

did you go down that tree asap from the start? During the year-long hiatus while 'govt. strengthens,' before you can get your PP malus removed, get yugo on board and then do other things. Buy your oil from Romania and don't bother guaranteeing Poland - it doesn't make them join the entente, and war will come anyway via CSZ...
Become facist France by throwing Versailles out the window (can't remember the exact name) then you can choose to form one of three factions, make your own, make one with Italy or side with Germany.
I did Czech Republic ASAP because I wanted to get strengthen govt.... also assumed joining would be automatic. Did Yugoslav after lifting "disjointed govt" a year later.
Does sound like I was just damn unlucky. Do I risk facing the Germans on my own or do I restart the game... (full time job and two young children, so every second of video game time has to be well spent).
If you have an autosave from the month before you finished the national focus, you could replay fom that.
Having done the Little Entente, I would not suggest fighting Germany alone. Re-starting doesn't take as long once you know when you are doing.
unfortunately the game force-feeds you friends whatever you do atm. One way or another, the world and his dog will come to your aid and saunter through your country without a by-your-leave.
Update: Started over. Czech joined, Yugoslav didn't. Either I'm very unlucky, the chances are a bit below 90%, or there's something I am missing.
Given your time constraints, I would wait and see if PDS releases the beta 1.1 patch in next day or so before starting a new game. It may fix a bunch of diplomacy and game issues.

I would very much keep a string of saves for every playthrough. You can do that with Iron Man saves as well. (Go to the save directory, copy and rename.) Most of the time, you won't need most of the earlier saves and you can delete them all at the end. But from time to time, you need an achievement that you lose due to RNG which makes no sense whatsoever to repeat a whole game just to re-roll 2 dices. As well, the game does have a good number of bugs. The more you play out of the box strategies, the more they show up. You need the saves to get rid of bugs from time to time.
