Having more than 2 empire titles isn't generally a good idea, though. Vassals get the "Desire X" modifier and you stop de jure drifts from that title.

EDIT: I mean more than one, no more than two.

Also you risk spliting your empire in faction/succession revolts. AI has a tendency to make a second empire title if they can which I never understood since they usually end up losing their second or first empire title after a few generations.

BBBbbutttt all that prestige!

No seriously never make a second empire title, this is a bad idea. If you must tho, just mouse over hispania's create button it should tell you what you need. You probably just don't have enough piety/gold/land. I don't think Hispania has any special rules.

Ok. I finally got it. This my emprire right now (Spanish version of the game, sorry) with a list of the 21 titles I can create. As you can see, none of the is the Hispania Empire.

I see. Pick any spanish province, click on the shield. Check "De jure" check mark. Start clicking on higher level titles until you hit empire Hispania. You probably are just missing a province or two.

Alternatively open title finder (left of character finder) and look for hispania.

It looks like a ruler already holds the empire title of Hispania, unless I'm mistaken about those yellow parts of Spain. If the title already exists you must usurp it to get it, and that is impossible if the character holds parts of the de jure empire AND is from another religion (looks like he's a muslim unfortunately). If that is the case you must finish the Reconquista and drive them off the Iberian Peninsula to get the title.

That little yellow blob in Lusitania looks like a Muslim Hispania. You can't form a title that already exists, nor usurp it if it's the only primary title of that tier held by an infidel with at least one de jure county in their realm. You'll need to eliminate them first.

Some empires have special additional creation requirements, usually requiring you to own a specific kingdom. For example, the Empire of Italy requires that you hold the Kingdom of Sicily, despite it not being part of the de jure empire. I'm not sure if Hispania has any special requirements, but you can hover over the Create option to check.

EDIT: Or it could already exist, but that doesn't look like the right shade of yellow.

This has already been answered. You can't usurp the landed title, which is currently a Caliphate if it is the primary title of a ruler who still owns land within its de-jure. That's at least three more wars for you I'm afraid.
