If you click once (in a city say) you create a new vassal with a random coat of arms. If you click a second time you change your States COA to that of the new vassals.

If I click that button a second time the Brandenberg crest will change to the new Baron's.
I don't think you can second click. Are you saying that the second click changed the shield from your title's arms to the Barrony's own Coat of arms? The first click should have done that. It probably took time for the window to refresh.
I'm not understanding this much either. You should right click on the holding picture and click create vassal or baron or whatever it says. I've never had an issue doing that.
Here's another example:
Note the two Apulia shields on the bishopric, bottom left.
After first click. Notice change of shield in bottom left.
After second click. Note change in Apulia shield in top right corner.

That indicates who owns that paricular holding, it would have changed automatically once you unpaused.
Yeah the first time it's still your own crest, whatever duchy that may be
The second time it's the crest of the bishop, showing that it's him who controlls the land now
It's indeed just a case of it not updating while unpaused, the same thing happens when you change your primary title
