as the title states do you need a hospital first? or is it a late game thing? long MTTH?

You don't need an hospital, and (if a province is safe from raiding/besieging/occupation ) it happens in a fair ammount of years (5-10 i think) .
Not so long, after all.
Also depends on terrain type ( Farmlands and Plains gain prosperity quicker than mountain/artic ) and of course good stewardship helps a lot.
But the most important thing is to keep the province safe from raiders and enemy armies.

Also, the "See the Realm Prosper" ambition will speed things up quite a bit if you stay at peace for five years. Also +1 Stewardship.
