I'm playing my first game with TRD atm, it wasn't until my 5th character that I managed to make it past age 35 and had spent about 50% of my time in regency. I also just had the "delayed dynamic" black death roll through in 1112 from the Alexiad start date. The Mongols arrived in 1150.

100 years into the game and I still haven't reached age 50 yet.

At first it was annoying, but now I'm enjoying it. It's a lot more unpredictable and it effectively makes feudal elective untenable and has significantly changed my play style.

I like it too, I'm not too sad now when I have a son when I'm only twenty, knowing he's going to become king at fifty when I finally die at seventy. Now I have as many kids as I can as soon as possible, because I don't know how long I, or they are going to live for.

Life finally has meaning again when it's short and you don't know how long you've got to make things work.

So far 1° ruler died from scurvy (careful where you raid now) at 53.
Second ruler die a "natural dead" at 56.
Third one died in battle at 52 years old.
The 4° and current ruler... well he became inmortal at 32 years old, but lose an eye in a treatmen of gout, that was a succes.

Anyway, aside from battles, I try to play "carefully"... but some of my son have die extremly fast.
One died of 23 years old, he became "the drunkyard" after 2 days rulling his duchy and died a month lather of food poisoning.

Half of my courtiers have died of chest pain, gout complications or flu. Average life expectancy is arround 40 years as long as they don't get sick. The rest is in the hands of their physician (near 20 skill, they will normaly will heal... less that 15... well good luck )
Hospital aparently are having some impact in the average health not only of the province, but also of the characters that live there.

Mine finnish rulers:
1st - 86 years (yup!), natural death.
2nd, his grandson - 70 years, severe stress.
3rd, son of second - 63 years, cancer.
4th, son of third - 70 years, severe stress.
5th, son of fourth - 40 years, great pox. He get it in his 18 or near, before becaming an emperor.
6th, son of fifth - 66 years, fatigue. He got his fatigue in his 8, living with constant treatment all his life; by console I saw he had tuberculosis, but it never developed. Also this guy was married on girl caming from India to help his grandfather with his quest for immortality (he died before he achieved it!).
7th and current - just 9.

Pretty much no changes in average life expectancy for my rulers, they still usually die between their 40s-60s.
From my Haesteinn of Nantes game:
1st - 63, Pneumonia
2nd, his son - 59, Measles
3rd, son of second - 61, Cancer
4th, son of third - 48, Cancer
5th, son of fourth - 43, Poor health
6th, son of fifth - 51, Comatose in bed
7th, son of sixth - 68, severe stress (worth noting the fact that he was one-eyed, one-legged and disfigured)
8th and current, grandson of seventh - 56. He inherited instead of his father because his father joined the Knights of Santiago. Unlike his father, wife and first son, he survived the Black Plague, but is currently infirm.

Additional anecdote for the perspective and the effects of RNG combined with feelings and perception: For me, Old Gods is a real character killer. 3 viking campaigns and in all I die all the time: Leprosy, poor health, duels, battles etc.
Second anecdote: RD seems for me not to lower life expectancy but fertility. All my women need way more time to get kids than before. It´s only a matter of chance, perception and what you make of it.

Another strong factor is map location - if you are near the silk road you are more prone to the various epidemics which seem to be something to be feared now.

So far my most venerable character in Reapers Due was a breton count. Even after losing his leg to cancer in his 50's he managed to reach the age of 77 before croaking. Ruled for 70 years. Probably one of my longest lasting dudes overall.

My first ruler had 7 children, and died at 50 with the eldest a young 17 year old. Gavelkind succession was a bitch.

In 4 years my dynasty was down to a single Queen, and Gavelkind is now just a free bonus to demesne limit.

My capital was on the Silk Road. Do not recommend.

I also had a character commit suicide at 93 or something ridiculous so his strong attractive granddaughter could inherit. Military education, Lover's Momento, the taking it easy modifier, and hunting focus giving a permanent 4 health, along with having a 35 learning guru keeping me healthy made me pretty much immortal.
