Naturally? 21 is the highest I've seen, but keep in mind that a lot of the pregnancies were forced by events, which bypass fertility.
Still, I don't think those events can fire when she's already pregnant.

Depends on the tier of the ruler. I think 12 is the max number of livingchildren an emperor can have (regardless of the mother).
I'd image that technically it is the same for a female ruler. But of course getting 12 children as a woman is much harder than as a man

Can you elaborate on this. Can lower tier titles have more children or less? What is this immortal trait, and how do you explain women and men with 21+ children (assuming they don't have this trait)? I've pretty sure I've seen Muslim men with more than 12 children.
~12 (maybe 13 or 14) for emperors. Less for the lower tiers. If you want to check this start with a Muslim emperors get yourself the max number of wives and see how many you can get. Show me the 21 and I'll tell you how it was done
Just remember: Dead children do not count here. If you are at the limit and one of your kids dies you'll be able to sire another one. Also this maximum can (somehow) be bypassed with bastard children.
immortal is something you can set in custom made traits (i.e. in a mod).
Someone should make a test. Breed super fertility women and men, and then make those women wives of a man (Gods, my English is failing me!).
There's a hard limit. Believe me. Confirmed by the devs and "empirical research".
Women become eligible at age 16, and infertile at age 45. Assuming they are constantly pregnant, this gives you a maximum of 348 months/9 for each pregnancy=38 kids. Assuming there's no hard limit, that is.
You forgot the possibility of twins. But there is a hard limit nonetheless.
I think I've sometimes passed the limit of 12 by having multiple women getting pregnant at the same time or having 11 children and then getting twins but I believe its correct that the normal limit is 12. I have sometimes had 12 children or so, stopped having children then many years later had a second batch of children. Not sure if thats because adult children don't count against the limit or some of them had died.

Otherwise, yes, 12 living children is a cap which can only be exceeded under a few specific circumstances (simultaneous pregnancies, event-induced pregnancies).
