Flashpoints have an "expiration date." If you do not arrive on the Flashpoint planet before it "expires" to take the contract, it disappears from the map.

Is there only a single opportunity to take a given Flashpoint or if you let them "expire" will they reappear at a later point in the game?

As a followup, once a Flashpoint is accepted and completed, is there a chance it will appear again?

For reference, I am playing in Career Mode. Thanks in advance ;)

They will pop up again if either they are not taken, or if they are failed.

If successfully completed they are done for that playthrough and will not pop up again.

Consider it a deck of cards with the cards being flashpoints.

Completed flashpoints are removed from the deck, everything else goes back into it.

So eventually you can do all flashpoints even if you could not initially take one on for one reason or another.
