What formable empire titles are there?

The ones shown in @Rikashey's screenshot, plus:
- The Latin Empire
- The Holy Roman Empire (French version)
- The Bulgarian Empire
- The Empire of Kalevala (Finland + Livonia)
- The Empire of Hungary (Hungary + Croatia)
- The Empire of Occitania
- The Empire of Ulm (Ulm + Ferrara)
- The Roman Empire

That's the Wendish Empire. In order to create it, you must:
- Have a religion of Catholic, Slavic Pagan, or Slavic Reformed
- Be Carinthian, Croatian, Pomeranian, Polish, Bohemian, or Moravian
- Be the King of Moravia
- Be the King of Poland, Pomerania, Bohemia, or Hungary

Also, it can't be formed if it's titular (i.e., if you haven't activated the "De Jure Empires" rule, which is active in the screenshot above but inactive by default) and it isn't already held.

I don't see it. Maybe you're thinking of CK2+.

There is a Poland-Lithuania empire, de jure after 867 instead of the Wendish Empire (if ahistorical de jure empires are on). For that reason it’s not in titular_titles but swmh_landed_titles.
