Repro:
I don't know the exact repro, but as far as I can tell, if you refuse the ultimatum to hand over one of your titles to a new ruler, the war doesn't actually fire.
I was playing as the duke of Aquitaine. One of the counts of the duke of Toulouse demanded that I turn over the duchy of Gascony to some random courtier. I refused, and I got a message that a War for a New Ruler had been declared. I raised my troops, but I didn't see any enemy territories highlighted in red. The diplomatic map didn't show that I had any enemies either. I even clicked my province and the province of the plotter, but neither of them showed any active wars, so as far as I can tell, the trigger that actually fires the war is nonfunctional for some reason. Unfortunately, I forgot to take a screenshot of the log before I closed the game. I'll upload the savegame, but since it doesn't show the log and it doesn't list the war as an active war, I don't know how helpful it will be
