Preface: In an ironman game as Korea, I was at war with Jianzhou with my ally Haixi. Upon winning the war, I took 3 provinces from them, and noticed I had enough war score left to vassalize them, so I did. Unfortunately, after the peace was concluded, Jianzhou ended up being Haixi's vassal rather than mine (which I really, really didn't want to do.) Haixi had one claim on Jianzhou lands - the capital, probably due to a mission - which they occupied during the war. The three provinces I took were the only others that had finished their sieges.
I can't go back and check, due to it being an ironman game, and it is a very difficult scenario to recreate, but I'm guessing the issue is that there was only one "X vassal" option displayed. It would technically not be a bug because it probably correctly displayed "Haixi Vassal" with their flag and I simply overlooked it because I'm just used to it being "*Player Nation* Vassal" and didn't even look at it closely.
My proposal is to always display "*Player Nation* vassal" first, in the event that it is possible to give a vassal to an ally - and if it is not a valid option for the player, grey it out, to prevent such confusion.
This is a problem that is probably rare, but incredibly irritating when it happens, particularly in an ironman game where you can't go back and fix it.
