I got a game over, not from death, but from losing my last titles and becoming unlanded. It took me a while to figure out it was my Liege's decision to give them to someone else... at least I think so, because the game didn't tell me exactly. So just a suggestion to make the Game Over screen abundantly clear as to what the reason for Game Over is, if it's not death.
