Won a war to capture a county and had the other side give up before I had captured the last holding in the county.
I was surprised to discover I didn't get this as well when the province changed hands, instead the minor holding stayed with a Muslim and was linked to a neighbouring emir.

It seems quite hard to conquer it because I can't declare war for that area, because I already control it.

Has also happened the other way, where I had seized some minor holdings which I kept after war finished, even though I didn't get to keep the primary holding in the province.

This is clearly how the game works now, but it seems weird.

It is often for muslim to have diferent lieges for holding in 1 province. You took holding's from one you declared war on, rest belong to other, or even they may be independent. Check it.

You can get control of the baronies inside a county you hold by using the "county claim" CB. If the baron is independent, you may want to check if they'll swear fealty to you first, so you can avoid having to waste troops cleaning out their holding.

This kind of stuff happens due to some count inheriting a barony in some other county and keeping it. Then, some years later there is an independence revolt or a war that simply replaces the top liege; titles underneath are still held by some lord ruling from somewhere else.

I don't have the game open in front of me right now, but I believe at a particular level of crown law you are able to revoke infidel titles without ramifications.

Yes, but only if they're your de facto vassals. Otherwise, you need a war.
