how easy would it be to modify the tab that holds the fort, trade post and hospital to include a university. This would be a special building that would allow for greater tech gains and you can send your children to get a higher education. Naturally it would e exspensive to build and possibly maintain. Perhaps even limit it to certain provinces in Europe and the Middle East.

Any thoughts?

I think it would look better if it were universities that have separate holding slots. They could still have hospital buildings as they are. The thing is, a province can have five cities, each of them with a university. I do not think this kind of institutions were that popular in the Middle Ages. In the same time, the hospital holdings are wonderful opportunity to create all round utility institutions. Also, I always thought that medical centres existed mainly as additions to churches or universities.
It would be nice to choose one technology branch to develop (3-5 levels, similarly to hospitals) in each province plus the hospital levels and utility buildings as they are.

I'd rather have the university be county-unique to be honest and have the extra subholding slot reserved for industry that is specific for different counties. Stuff like iron/copper mines, vineyards, regular farms, fishing industry and that sort of thing. It would improve the "building tall" aspect that was added in RD even more.

I'm not sure that the university building represents an actual university. In this time period, most countries had a single-digit number of universities; England, having 2 in close proximity, was quite remarkable, for example. I like to think that the university building, at least at lower levels, more or less just represents a local system of trade schools (thus the economy tech bonus, when a real university would be culture)

You can't add sub-holding types (neither holding types).
You can use use buildings, province or holding modifiers to achieve your feature though -looks much less cool of course-

Some time ago I suggested Universities to serve various functions.. suggested them as building and function like i.e. palace..

but that would require deeper touch than modding
