Two problems - vassalising and annexing

Playing as Brandenburg I stumbled upon two problems regarding vassalising and annexing nations:

1) The Palatinate refuses all vassalisation offers. We have an alliance, relations of +200, same faith, royal marriage and they keep refusing every time I offer it. I've never had such a problem before.

2) I can't annex Kleves - they have been my vassals for over 20 years, we have +200 relations and they are part of my alliance, however I can't even offer them an annexation - that option in the diplomatic screen is greyed out. I have the same problem with Tyrol.

I'm pretty confused so I'd appreciate any help.

Late patches make diplo-vassalization very, very, very unlikely unless you are very, very, very more powerful than the prospected country. Force-vassalize your way around that problem.

Are you sure you meet every requirement ? There's a Diplomacy FAQ in the FAQ subforum that expresses all concerns.
Key elements I see missing in your description are :
- common borders ?
- compatible religions ? if they're catholic and you're protestant, you won't be able to DA them until the Edict of Tolerance mega-event triggers
- are they at peace ?
- do you have a diplomat available ?
- is your economy better than their's ?

Ah silly me, I don't have a land border with Kleves nor Tyrol, I forgot that Thanks for the vassalization explanation
