If I understand it correctly, the value displayed in the Military tab is a sum of all units monthly upkeep, assuming the slider is set to 100%. In the test game I started, this appears to be the case. I have 8 regiments, each costs 0.25, a total of 2. So far so good.

I also have 15 big ships, each costs 0.24, which should be 3.6 monthly. But the value at the slider shows only 1 ducat maintenance. How come? Is there some modifier that I'm missing?

Naval/land slider, NIs, decisions, trade-triggered modifiers, and buildings all affect the actual upkeep but they clearly are not all included in the tooltip when you mouse over the individual units. As a counter example, I have a Bavaria save where my navy consists of 40 large (.14 each), 109 light (.05 each) and 20 transports (.00 each) which adds up to a bit over 11 yet mousing over the naval maint slider shows a base of 47.81 (before my over the limit penalty is applied).

On the same screen my land is 231 * .22 (46.62) + 68 * .30 (60.4) + 4 * .52 (2.08) = 109.1 but regular cost is listed as 74.80.

Note that all land units were built in provinces with land 1-3 buildings (or more but higher has no effect on cost) and my navy was all built with naval 1-6 except for 2 captured transports so all should cost the same.

Conclusion: the mouseover unit cost is just flat wrong (on both land & naval).

Well, if you go by the mouseover tooltips it does. Carrying on my previous example and removing the n5-6 buildings changed the mouseover for large ships from .14 to .28, light ships changed from .05 to .11. Interestingly, the value on the military page did not change. Clearly something is not working properly.

As others have said, it does. Therefore it saves money on maintenance to build all army buildings in a few select provinces and only recruit your troops from those provinces (unless at war and you need a lot of troops fast).

I'm afraid you're right. I guess I'll never know for sure how much a new ship or regiment will cost me until I get the first monthly bill. :)

modifiers apply to all units, even those built before the buildings, for construction and maintenance.
And we can tell this by looking at a savegame file; each regiment/ship has a home province listed but no indicator of cost or when it was built so it can only be based on the current state of the province.

The relation between build cost and maintenance cost is shown in the defines.txt file:
