Playing as HRE (Burgundy), have passed all reforms except last one. When I try the event does not fire. I am playing divine wind with most recent patch and have 56 Imperial Authority.

Has this happened to anyone? How do I get it to fire?

All HRE members vote on reforms (not just electors) and you need a majority to pass. On the EU3 wiki there is a thorough explanation on what factors the chance of voting yes depends, but some of the more important are:
1) relations higher than 100 improve the chance for voting yes, lower than 100 increases the chance for voting no. Get as many HRE members above 100 relations. Above 150 there is another boost.
2) A member at war with you will basically never vote for you.
3) Having other religion increases the chance for voting no greatly.
4) Having your infamy > 10% of your limit increases the chance for voting no greatly.
5) A HRE member with 6 or more provinces will have a greatly increased chance for voting no.

To also add: theocracies care more about being right religion, and unlike the vassalization vote, if you win the total, objecting or not doesn't matter. There's no need to waste diplomats on harder cases, as long as you can get a majority of states.
