Titular Title name error [2.7.1]

I forgot to put the version is 2.7.1 in the title and that there are no mods being used. I have all the DLC currently out. I shouldn't make these at 1 in the morning.
I have a save that was made just a few days before and the Patrician keeps doing it over and over. The shield seems to be the same as the Duchy of Normandy, which has already been created and claimed.

Seen this as well, no idea how to reproduce it. I saw it happen with the HRE's younger sibilings in a gavelkind inheritance. I didn't notice it soon enough to have a save, so you should upload yours.
How do I upload a file here? I tried the "upload a file" but it tells me the uploaded file does not have an allowed extension.
This bug has been around for at least a year; I've never figured out exactly what causes it though. There don't tend to be tons of them, but a game will usually end up with at least a few (if you scout for them).
It's my first time really encountering a bug like this, but it's also my first game to pass over 300 years.
I've just seen this myself as well. In my game, it uses the flag of Scotland (Pictland), which is still owned by a Pictish king, albeit one who has mostly migrated into England with only three provinces left in Scotland. Let me know if a save-game would be helpful...
