Title Usurped Pop-ups

I keep getting title usurped pop-up windows for Bishophrics when the message settings is set for Close and unlanded family members. The people that usurp the title and the person who loses the title have been lowborn characters not even of my dynasty. And as far as I can tell the bishoprics and the characters who usurp them aren't even my direct vassals.

Can someone tell me what's going on? Because I want to be notified when family usurps or loses a title, but getting SEVERAL bishophric usurp pop-ups a month is going to drive me nuts.

Also, in my HRE I have vassals fighting each other even though Vassal War Declaration is set to External.

It's a bug.
The bishop converts and gets replaced.

Also, in my HRE I have vassals fighting each other even though Vassal War Declaration is set to External.

-Are both de jure HRE ?
-Plot to revoke or just revoke and consecutive tyranny war does not count

Vassal Kings fighting each other. So they shouldn't be able to plot against each other except for the standard assassinations right?
