Yes. That's why that approach was chosen (generally players don't like being overly restricted so it's avoided when possible).

There are fewer duchies..howver they aren't non-existant now.

However, i think it might be a good idea to have something like the "i want to be..." events for counts who you can be given duchy titles relacant to them (assuming your a king). Otherwise for humans, other than for your son, there's no reason to hand out duncal titles, which isn't historic, ie that only sons get them.

Ducal titles when given out give the same +0.2 to reputation / month modifier as giving out a county. If I run out of county titles to give out, I do start handing out duchies, but you're right, they are last.

Considering how duchy titles give monthly prestige for kings, I usually give them all out, unless it would be waste of badboy (ie, I have less than -0.2 monthly loyalty penalty from reputation).

Playing 04/08 patch. I don't see why Kings should have any reason for not giving every duke title they can according to the new rule. My liege King of England created the title of Cornwall but refuses to grant it to his count brother. The same was true for France with Orleans and for Germany with Franconia.
I also have the impression that Kings don't create titles when not personally holding two thirds of the area, and that dukes don't do it either for those who could (Poitou, Toulouse, Catalonia...), without really knowing what to think about it. They should do it in their own interest (human player does it), but it may seem only fair that an ambitious count of theirs should be allowed to try (especially when you are the ambitious count).
The other thing I am unsure about is wether a King should be able to grant duchies to counts who are not his direct vassals. I don't play often as a King so I don't remember it this is actually possible (don't think so), but it might be an idea.
