I don't mind the sound effects but some of them were way to loud. For instance, the trumpet sound when you click your coat of arms. I downloaded a wave editor and reduced the amplitude of several effects such as the trumpet, the marching sound, and the catapult during sieges. Now I can turn up the sound effect option and hear them all without being blown out by one. I didn't even realize there were as many sound effects as there are till I did this. The throat clearing one is strange but at least it didn't blast me out (Didn't even know it was there till I was able to turn the sound effects up).
I tried the same thing with the music and an MP3 editor but for some reason changing the amplitude doesn't reduce the base volume of some numbers. Hence, I have to choose between a music setting of 3 or 4. 3 turns the music off, 4 has some compositions playing to loud.
Maybe in CK2 (or EU3, Vic2, etc...) the sound could be moderated a bit? I have the same problem with all Paradox games and the music volumes. It's too loud down to 4 then it's off at 3. I'm using a Soundblaster Audigy with JVC speakers, my friends have the same problem so I know it's not just me.
