It entertains me that it tends to be the period after CK2 where most rulers go title mad, but in the actual medieval period, with a few exceptions (looking at you Byzantium), titles tended to be relatively simple, such as Ӕthelred, tend to read "King of the English", or as "Emperor of Britain".

Because of centralization efforts the rulers ended up keeping alot of titles they'd otherwise hand over to vassals. Also this is the time they started to compete which of them had most meaningless titles and honours.
