I want to make the Coptic Pope the Duke of Alexandria, but I can't because theocracies can't have two duke level titles. This is hardcoded and I can't change it myself. Please make it so that religious heads can have a titular title as well a de jure one of the same level.

The Pope, and I believe most theocracies can create new titles for themselves if they meet the requirements, but you cannot explicitly grant extra duchy or higher titles to them once they've already got the one. The Caliph uses the Iqta government so he's not a theocracy. As it stands there is no specific way to give theocracy vassals extra duchies except by giving them lots of counties (which isn't always possible) and hoping they make the titles, or using the give_title console command, which is prone to awkwardness.

Catholic Pope and Ecumenical Patriarch are king level titles, so they can have multiple duchies but only one kingdom. Caliphs can have multiple titles due to the Iqta government. All other religious heads have duke level titular religious titles, so you can't give them de jure ducal titles.
