I wish i was lying about what im about to say, but with this DLC i had a game last for 4 years and 44 days, I only had 3 khans! (I plan on making a AAR on my next play through because the way diseases are, it needs to be tonned down like A LOT Horselords are unplayable due to the rate of disease and no way of getting medical hel) so anyways.

My First Khan died within 2 years into his rule from Sudden Death Pneumonia (and this is extremely common in horselord nations) which we will now call "SDP", since there is NO alert that your character has it, they just die from it. He had one son, his heir (died within the first year of the game SDP and his wife died of Gout no children) and a daughter SDP. Also his wife died due to poor health (i wonder why), same with his second wife

So his brother took over as Khan, and that was fun because he raided like 2 areas, subjugated a splinter group of Khazackstan and like a week later his wife died over getting into a fight over food. (< thats a legit mechanic in the game, I am not lying to you thought I wish I could for comedic affect) and his second wife SDC since i got the princess of bulgaria. Then he got smacked with like your usual Coughing, Sweating, Vomiting, Chest Pains, Gout died. He ruled for 2 years no children cause either he was a hermit male without being chaste or celebate or his winky didn't worky.

Now I know your thinking "Why not get a medic or something?" BECAUSE YOU CANT! And there is no option to get a court physician or a hospital in Nomadic kingdoms which makes it impossible to play. So get ready to constantly lose Khan Title. But then a second rebuttal would be "did you get concubines?", Yes i did and they never got pregnant AND if they did my ruler had died by the time they gave birth and this is with me having the family way of life perk selected, wanted to try seduction but NOBODY lives over 5 years in a Nomdic kingdom

next up is my final 44 day living Khan, my Khanum who had lost turkestan for me turning the Yabugid faction into Bozok, was a 9 year old little girl and my original Khans half sister. I raided one province, seiged it down, saw that theres an event when you take the first holding, had fun, got coughing and chest pains since i got them like within 3 days of each other, played 2 more days with those 2 diseases, didn't have fun, SDP. Lost the game.

If your playing a horselord nation, please post your experiences like mine because now that i reflect, its just funny!

honestly was kinda hoping for a "horsemen of the apocalyopse" event chain in the supernatural events letting you do something nasty to a neighboring feudal realm you otherwise wouldnt be strong enough to fight

and historically, werent the nomadic kingdoms LESS likely to fall prey en masse to the diseases? i mean no reliance on staple crops, they dont enclose themselves in tight groups for long periods of time, they dont share a single drinking source for long periods of time, they dont dispose of waste in a nearby centralized location

all in all many of the weaknesses of settled civilization were the strengths of the nomadics, the costs/benefits of the 2 lifestyles were kinda exact opposites historically.

would be nice if just to reflect the non-centralized living/waste/food-reliance, as well as the ease of which dead-disposal could be done (no mass-collective for mass-graves), shouldnt nomads just get a flat percentage disease-resistance? or at LEAST a building in their capital holding to up it?

When you said no medical help, are you referring to the "search court physician" decision or that you don't even have the option to appoint one from the "minor titles" section (any character with at least 15 learning can be appointed physician, if you don't have one the option will be "greyed" or will tell you that no valid candidate is present)

With the invite holy man to court a manage to get a "medic" (aka character with 15 or more learning) playing as a tribal ruler.

Check if you can get a medic that way... if not, them nomads are in a very precarious situation :/

Deseases are somewhat OP at the moment, they will get toned down I guess. My characters somehow were okay most of the time, but everybody else was getting ill and dying horribly all the time.
