The current flag of Sicily (SIC) is like this:
While the current flag of Two Sicilies (TTS) is:
The first one is a flag of the Kingdom of Sicily (present on the flag of Spain), while the second one is an old flag of Sicily (SIC), closer to the modern one, used before "Emperor" DLC. After that DLC came out, flags for these countries were swapped.
My issue is the following: after Sicily Proper was lost to Aragon, capital of the Kingdom was moved to Naples, so Naples (NAP) tag in the game actually represents the Kingdom of Sicily like it was in real life, even without an actual Sicily. It's already represented in the game: Naples have cores on Sicily and Malta. While Two Sicilies came into existence only in Y'1816 (almost ending date of the game), after both Sicily and Neapolitania were juggled between France, Aragon, Spain and Habsburgs multiple times.
So I think that in the game Two Sicilies should represent the Kingdom of Sicily - the old unified kingdom that esixted before, thus having the first flag. While the Sicily Proper should have the second flag, being a somewhat other entity.
What do you think about it?
