Long story short, the order housed knights of other orders who had to be removed due to their leprosy. They were somewhat similar to Baldwin the Lepper back when he could still fight. They aren't probably talked about much, but they did participate in some of the important battles — not necessarily being all leprous themselves, but leprous knights were transferred to that order.

Perhaps there could be some events to move leprous nobles to that order of St. Lazarous to live out their days? This could include abdication, although that could be a little far on the 'alternative' side.

The order would be mostly for flavour and for the blood and tears and the sort of desperate situation that prompted the militarization of hospital orders. Those were people who were dying either way, and death by the sword or arrow was the milder kind. They could be small in numbers (even very small) and perhaps afflicted with an organic, physical weakness but with a propensity to lead difficult charges and have less regard for personal safety than the typical knight, even less than Templars and Hospitallers. They could perhaps mostly just attach themselves to the larger Templar or Hospitaller stack (as a very minor portion of it, numerically) and be unavailable for normal hire by normal characters.
