I had something confusing happen and I'm trying to figure out if there was maybe something I missed. This is with version 2.1.6 (TTXL).
With the Iron Man save changed from monthly to every six months (which I love), I decided it was time to give Iron Man another play-through. My goal this game was to play all the way from 867 to 1453.
I started as King Sigurder "Snake-in-the-eye." His descendants went on to usher in a nordic age, with a Scandinavian Empire whose land consisted of both its native territory and the empire of Brittania's de-jure kingdoms drifted into itself. An ambitious kinsman went on to conquer Aquitaine, and another Brittany. Both were incorporated into the empire. My latest project had been conquering the duchy of Sicily from the Byzantine Empire to establish a nordic presence in the Mediterranean.
Fast forward to the reign of Haukr the Great. As his forefathers, he held both the title of Emperor and the Fylkir:
Catholic Europe was starting to crumble. This led up to a crusade declared against me in an attempt to reclaim Aquitaine. I'd been looking forward to this confrontation for some time, and had four armies immediately answering with blood and thunder: one advancing on France from the shores of Normandy, one landing on the Bay of Biscay to reinforce Aquitaine, one force descending from Denmark into German lands, and a force to lay siege to Rome and afterwards Italy.
And then the Emperor died.
My first concern was managing this massive war with the potential of internal schisms as the realm was (supposed to be) passed onto his youngest son, Rune, according to Ultrimogeniture tradition.
But then I noticed my sea of nordic blue was gone. The game was paused following the death screen, and I tried to survey what I had. Rune held only a strong claim on the Empire and Fylkirate, despite both titles having been listed for him as my heir. What's more, he was only a courtier, sitting in Monreale. When I unpaused I was all of a sudden Emperor Haukr's second eldest, Jens: King of Denmark. Rune still lived, without any titles, and the empire was still nonexistant. Both the Emperor of Scandinavia and the Fylkirate are listed as "No Holder" with Rune as the last to have the titles. The reformed nordic church has no religious head.
As the subsequent kingdoms of the empire (Denmark, Norway, Sweden, Finland, England, Wales, Scotland, Ireland, Brittany, and Aquitaine) were all under the greater title of Emperor, and Emperor Haukr happened to have five eligible heirs lined up, I'm not sure how the titles just disappeared. It's 1077 and I feel like all the progress I worked on magically went away.
I still hold my demesne in Denmark. Despite my retinues being erased, I can still likely claw my way back and reform my empire. But, if the titles can just disintegrate, why bother?

Here's the history of the Scandinavian Empire title. Rune is listed, though he lacks the empire border suggesting he ever really held it.
