So I just started an Ireland game earlier today to try out Reaper's Due with the epidemics and diseases turned up to the maximum. In the first 20 years of the game, about 1/3 of all deaths had the little green skull.... I lost a wife to slow fever, than the next wife and a son to camp fever. Then the next wife and a daughter to smallpox. (I survived my bout with smallpox.) Then the Black Death came, and for the first time, I went into seclusion before it hit my province. Two people stole food, and I kicked them out. Both of them died of plague within a few months.

I'm enjoying the game with insane diseases. For one thing, it means the early gavelkind inheritance trick of only having one son is far too risky to employ. (Best option is have two sons and hope one of them either dies or betrays you shortly before you die. Best way to ensure this is to make them ambitious if at all possible and then land them.)

I have a Dublin game that i started just as the DLC came out and though the rate of death is increased, I didn't really notice the effect, even as a tribal and even as a Feudal Lord.

And then I tried a Smaland game. Every single ruler I had lived long enough to pop out a single kid then die. Immediately. If i didn't turn off regencies i would have spent 2/3rds of my time unable to do anything. It was absurd, to the point where I abandoned save. And it wasn't due to epidemics, I had kids coming of age with cancer or lists of horrid diseases it was absolutely nuts.
