This was discussed multiple times, most recently in the bugreportforum.
The cores on Skåneland (Halland and Skåne in hte game) are due to Sweden redeeming the pawn on them earlier in the 14th century. They were administered by Sweden for quite a few years in the 1300s after the danish kings bankrupted themselves and pawned most of their country (and a Swedish king redeemed the particular pawn for Skåneland).
Apparently the Swedish core on Jämtland is due to it falling under Uppsalas archbishops (imho a much weaker claim atleast before the reformation but that's the only reason anyone's been able to find as far as I know).
