Did you move around the date or looked around the different starting bookmarks? That screws up the game and that's why you got a 289 year old as a starting ruler.
Could you give us a screenshot so we can see if there is anything else wrong? And one of the tooltip from mousing over your ruler too?

I've seen a post a while ago that this happened to a re-released nation. Before the tick, the king in place was the same one that got fully annexed 150-200 years before. I don't know the particular conditions to reproduce this nor if it's still possible. Doesn't seems to be this case either if it's at start date, but i've heard of these old leaders before.

Sounds like a CK III bug lol there was a 140 year old leader around because one of the modifiers was broken and incorrectly calculated death chance.
prob your events didn't load. there is an issue when no pop up events happening and they are stacking. if you reload the game, all of the event slap you at the same time. king's death also happens in an event so that might be why. if you reloaded the game and the problem still exists, idk what to say.
