First off, I would like to thank Paradox for developing the level of historical games that you have done. And, I think this is one of the best games that you have developed.

I know that the Republic DLC is being worked on right now. I would like to cast my vote for ideas that could be developed further in CKII.

1. Making pagan countries playable with certain modifications to gameplay for them would be an interesting addition.
2. It has always been my personal wish that I could play a country with a religious leader, especially Papal Rome; that would probably require some religious decisions being added and expanding the scope of Papal Rome's influence and its interaction with catholic countries.
3. A bit outside the box, but what about adding a DLC that would allow playing one of the American (south or north) tribes; The Sunset Invasion DLC made me wonder how it might be to play the Aztec or Mayan tribes.
4. The player should have more freedom to change his/her religion in the game.

Also, the Antipope really should have more scope related to his existence. And, the Pope in Rome should be excluded from having any influence related to Piety in a country with an Antipope. Currently, if a holding takes on a heretic religion, the pope in rome will make demands with a piety penalty; if an Antipope exists, that should be excluded.
