This happened in my most recent game. Married my son to a duchess and they had a daughter of my dynasty. Her mother died quite young most likely assassinated. Now the daughter is in power but here comes the bug. Her direct heir is person from my dynasty that is far removed from my main family and the inheritence law is agnatic-cognatic primongeniture and that would make my son and the current duchess father the heir. Funny thing is that the next in line after the random dynasty member is me. The third would be my brother when by the law it should be my son (duchesses father), me (grandfather) and my oldest son. Now it goes random dynasty member, me and my brother. The duchy is Valois its currently in the county of Rosello.
