Please explain your issue is in as much detail as possible.
I've been struggling to win wars for some time now, and I think I've found the cause. When ships are lost in combat, they are not all counted against war score. In one case, I lost 9 ships (only 6 registered as lost) while destroying 13 of the enemy's ships, but none of their losses were counted (see attached screenshot). To confirm I disabled all mods, loaded up another game, and confirmed that ships destroyed are not counting against war score as they should be (see other screenshot where 7 losses occur but only 1 is recorded).

This is possibly the most game breaking issue I've seen so far as it makes wars, which are already difficult to win, that much harder.

Steps to reproduce the issue.
Engage in combat where both sides take losses. Not all losses will be counted against war exhaustion.

While on the topic of war exhaustion, it seems really bizarre to me that bombarding a planet into dust and enslaving the whole population contributes almost nothing (just the value of lost troops) to the enemy's exhaustion. It almost makes me feel like no having ground defenders is a better strategy, since then you take less exhaustion and can then capture the planet back later once their ground troops have moved on.

It may be just a visual bug. I have seen the war screen telling me my enemy lost no ship, yet a significant amount of war exhaustion is added to their side in the same battle.

The reason I don't think it's just a visual bug is because the ships are being destroyed, and the peace calculation is definitely based on the exhaustion that's shown on the war score screen. There may be an error in the war score calculation, but I don't think it's just a visual bug.

I thought so as well but I just fought off a fleet that lost 12 corvettes vs my 4 and I ended up with 4% exhaustion and they only received 4.5%. This is very far off from a cost perspective, not to mention the morale hit they should suffer from having to retreat and repair, as well as losing all those personnel.

I do have evidence to back my claim. Look at the screen shot I have attached. You could of course blame the mods that has no modification to this screen for all the error, but, well, if that is the case, why not blame cosmic radiation for corrupting my hard drive? These two explaination as possible as each other!
