Please explain your issue is in as much detail as possible.
Honestly, I'm not entirely sure why this is (/isn't) working the way it's intended. I declared a war to Impose Ideology with the intent of making a weak, easily-controlled buffer nation between me and an antagonistic neighbor (we're both militaristic, but I'm fanatical spiritual while they're fanatical materialist). I've got long-term plans to vassalize, absorb, and then break the territory up into a bunch of microstates so I can put Recruitment holdings in all of them and get *all the pops*. Turns out my neighbor has some friends, but they're really far away and also kind of beat up from *their* war, so I kinda ignored them, took over a couple of sparsely-populated sectors, occupied all the planets, and then gave one of their core worlds a taste of some Javorian Pox figuring it would accelerate their war exhaustion, all before this nation's supposed protectors could get into position to counter my offensive. It worked, they're suing for peace, but when I accept the Status Quo peace it's not creating an independent nation at all, all the occupied systems and planets are reverting back to the defender.

DLC: Everything official except Overlord.
Mods: Tiny Outliner and Light Borders + Swapped Colors + Star Pins (disabling them before accepting the peace offer didn't work, but I didn't try re-fighting the war with them disabled -- unless one of them is *really* broken I can't imagine them having an effect, anyway)

Steps to reproduce the issue.
I can't figure out why it's happening so... no idea. My best guess is that there's something with either the guaranteed independence or vassalage, but I did some Googling around and I'm pretty sure I'm meeting all the conditions to get my future puppet state and I have no idea why I'm not.

I've fought a few more wars, including Ideology wars, and it does appear to be the case that vassal states are just plain immune to getting split up into multiple factions.
