Please explain your issue is in as much detail as possible.
Twice during this game I have declared a casus belli to turn another empire into a vassal and then settled for a status quo. This has both times generated a new empire out of my occupied systems, as it should. However, this new empire is not a vassal of mine and is an immediately legal war target. More importantly, all defender belligerents other than this new state become my vassal, ie everyone that I have failed to vassalize if the game were functioning correctly. Sadly I am playing with an ironman save file so I cannot fiddle with the game to try and reproduce it.
I have attached the save file. I just ended a war with exactly this outcome (second war with this outcome and bug of this playthrough) and the novel empire created (the one that should have been my vassal) is in the south of the map and is my only current war target. (I just started this war after the glitched ending to the last war)
edit: Sorry, a note. Your questionnaire about which DLCs I owned didn't include all of them so I just checked all the boxes. I own all DLCs.
Steps to reproduce the issue.
(1) Starting a new war with vassalization casus belli, (2) fully occupying some systems with at least one colony and then (3) settling status quo should do the trick.
The only variables that I can think of are that it maybe has to be (a) a war to make the enemy into a Subsidiary-type vassal and/or (b) there must be multiple defensive belligerents, but I'm not certain.
