Rebellion against Determined Exterminators creates a blank empire with no name & personality

Please explain your issue is in as much detail as possible.
In my game, a rebellion happened against Determined Exterminators. The empire that rebelled has no name at all, and has a Despicable Neutrals personality. I suspect such things shouldn't happen.

Update: the unnamed nation won the war for independence, and some years later somehow became a part of the original Determined Exterminator empire again. No idea how, I haven't noticed any wars being declared (though maybe I simply didn't see it). However, some time later one planet in DE empire had 11% stability and rebelled again, and yet again created an unnamed empire with no civics and Despicable Neutrals personality. Seems to be a consistent behavior when there's a rebellion in a DE empire.
